Certifications: BLS, MSW Must-Have: Must have previously worked at
least 1 travel assignment Description: The candidate must be a
licensed social worker who has taken and passed the exam.
Experience in LTAC, rehab, and short-term acute care inpatient
medical social work is required. The candidate will be responsible
for coordinating services to home care and equipment following
discharge. Documentation of pertinent information to facilitate
care coordination is necessary. Participation in care conference
activities is expected. The role includes referring to rehab and
long-term care facilities as well as other disciplines/resources.
Effective and efficient supervision of staff is required. Knowledge
of adult protective services, child protective services, emergency
detention, guardianship, power of attorney, adoption/foster care,
and drug/alcohol use and treatment options is essential. Discharge
planning and knowledge of advance directives are important
components of the job. The candidate must chart using Cerner.
Business casual attire is required, including black pants and an
Azalea top. Flu vaccination is required with no exemptions. The
candidate must not have any more than 7 days of time off during the
13-week contract. There is a preference for no time off in the
first two weeks of the assignment. Two supervisor references are
required for the application process. Local candidates must be
within a specific radius, and the address must match their driver's
license when determining local status. The candidate must have a
valid state license in hand, or a compact license, depending on the
location. All submittals will go through a review process. An
interview with the manager may or may not occur. If the candidate
has questions, they may reach out to the account manager.
Completion of modules is expected prior to start, and any
additional hours needed for modules will not be billable. Modules
are typically recorded via transcripts.
