Description:The Substance Abuse Counselor provides direct
counseling and case management services to patients, consisting of
individual, and group services, including assessment,
individualized treatment plans, individual and group counseling,
and discharge planning.Duties and Responsibilities:Maintain
caseload per State RegulationsAssures incoming patients have
psychosocial assessment and an interpretive summary according to
the timeline in the ARS Policies and Procedure ManualCompletes,
with patient participation, a Preliminary Treatment Plan according
to the timeline in the ARS Policies and Procedure ManualCompletes a
Treatment Plan Review and update for each patient after completion
of the Comprehensive Treatment Plan according to the timeline in
the ARS Policies and Procedure ManualPrepares a discharge
transition plan for all patients according to the timeline in the
ARS Policies and Procedure ManualProvide coordination of care for
patients as neededAdherence of 85% patient engagementSatisfaction
rating of 85% or better in patient satisfaction surveysBe
proficient with State Licensure Standards, CARF Standards, 42 CFR
and HIPAA standards and Company policiesAdherence to Mission and
Vision of the companyRequirements and Qualifications4-year degree
in a social science discipline with a CSAC, CSAC eligible
candidates will also be consideredMaster's degree in social work,
counseling, psychology, marriage, and family therapy and one year
in the field. (preferred)CSAC, LMFT (Licensed Marriage & Family
Therapists), LPC(Licensed Professional Counselor), LCSW (Licensed
Clinical Social Worker), LSAT, or LRIC license preferredPay
