Job Description:

The Service Coordinator receives general supervision and direction from the Administrator/Manager. The Service Coordinator is responsible for ensuring that residents are linked to the supportive services they need to continue independent living at the community. The Service Coordinator will comply with established policies and operational procedures. In the absence of the Administrator/Manager, the Service Coordinator will accept advice and instruction from the Regional Director.*Duties and Responsibilities: * The Service Coordinator is responsible for the development and implementation of wellness-related programs, services and activities that promote, enhance and maintain resident health and independence. Specifically, the Service Coordinator:1. Provides general case management and referral services to residents needing such assistance.2. Establishes relationships with agencies and service providers in the community. Develops and updates a directory of community resources for use by both community staff and residents.3. Refers and links the residents of the community to local service providers, including personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, legal or other advocacy groups. Develops case plans in coordination with assessment services in the community and the management staff.4. Educates residents, families, and staff on service availability, application procedures, client rights, etc.5. Monitors the ongoing provision of services from community agencies to ensure they are appropriate, timely, and satisfactory, and manages such services as appropriate. Provides case management, i.e., evaluation of social, psychological, and physical needs and the development of a service plan, for a resident when such service is not being provided by the general service community. Meets with service providers as needed and as appropriate. Keeps the case management and provider agency current with the progress of the individual if resident has provided required consent/confidential release.6. Sets up volunteer support programs with service organizations in the community. Connects individuals to internal, as well as external, volunteering opportunities.7. Helps residents build informal support networks with other residents, families and friends.8. Educates residents on issues relative to service coordination. Obtains outside speakers as necessary; minimum of two per month.9. Provides mediation services for resident disputes, as appropriate, and seeks assistance from existing community resources when needed.10. Establishes positive communication with management, residents, and the community-at-large through a variety of ways, including education programs directed toward building self-esteem, home management skills, communication, etc.11. Encourages residents to be proactive in meeting their social, psychological and physical needs.12. Advocates and negotiates on behalf of residents for adequate, timely and cost effective provision of services.13. Establishes fundraising efforts to offset expenses for equipment and services for resident services and programs. Minimum of two fundraising events per year.14. Assists residents/families when a higher level of care may be necessary.15. Assists management in identifying residents who need assistance and provide contacts to local agencies.16. Maintains individual resident files, either electronically or paper files, that will contain documentation to include intake information, service termination information, review and follow-up, and resident/family meeting notes, and keeps resident files secure to ensure confidentiality.17. Completes reports in an accurate and timely manner providing copies to the Administrator/Manager, Regional Director, Quality Assurance team, and government agency (where applicable).18. Pursues avenues for additional services through private local, state, and federal sources.19. Educates staff on issues related to aging in place and service coordination to help them work more effectively with the residents. Minimum of four trainings per year.20. Provides service coordination topics for the monthly newsletter and provides in-service training and speaker information for the calendar of events.21. New service coordinators must complete a minimum of 36 training hours within 12 months of initial hiring. Thereafter they are to complete 12 hours of continuing education per year as outlined in the HUD guidelines.22. Refers all management and facility questions to the Administrator/Manager.23. Provides on-call emergency services as scheduled.24. Observes and protects residents' rights including rights to confidentiality, privacy and dignity.25. Practices safety procedures and complies with established reporting of accidents and injuries.26. Performs any other related duties that may be assignedJob Type: Full-timeSchedule:* 8 hour shift
