Responsibilities: Assesses patient status by obtaining health and
mental health history through patient/family interviews and chart
reviews; assesses presenting illness, risk factors, family history,
psychosocial situation, and cultural factors; and performing
appropriate physical examination. Integrates data to determine
diagnosis and therapeutic plan, including identification of any
health risks. Develops and implements treatment plan by providing
evidence based and/or professionally respected interventions.
Monitors progress during treatment. Instructs patient/family
regarding medications and treatments. Educates patients regarding
health promotion/illness prevention. Seeks other information as
needed, including consultation with clinical leadership, physicians
and other clinicians, for evaluation of illness. Recommends
appropriate community resources to meet patient/family needs.
Communicates appropriate case management information to other
professionals and community agencies. Prepares documentation for
medical records within 72 hours including updating patient medical
chart by posting assessment and test results, diagnosis,
medications, and treatment in written/computerized manner.
Participates in peer reviews, chart reviews, staff education,
clinical guideline development, and other continuing education and
quality assurance activities to demonstrate compliance with
standards, regulations, policies, and procedures. Collaborates with
multi-disciplinary team as needed in managing acute and long-term
medical needs of patient. Provides monitoring and continuity of
care between visits according to treatment plan including triaging
patient calls/e-mails within 24 -48 hours. Competencies: Effective

concentrates his/her efforts accordingly. Avoids wasting other's
time. Achieves goals and meets commitments. Problem Solving - Asks
good questions and probes all sources for answers; sees underlying
or hidden patterns; looks beyond the obvious and doesn't stop at
the first answers. Uses rigorous logic and methods to analyze and
understand why problems occur; generates and implements creative,
cost effective and realistic solutions. Clinical Skill - Skill in
modifying methods and techniques of clinical social work or
counseling to provide appropriate therapeutic interventions and in
initiating appropriate emergency response and investigatory
procedures. Cultural Competence - Understand and respect the
cultural beliefs, values, and backgrounds of clients. Demonstrate
awareness of cultural biases and assumptions to avoid imposing them
on clients. Ability to adapt therapeutic techniques to align with
the cultural context of clients. Ability to establish/maintain
effective working relationships with patients, clinical staff, and
the public. Experience and Education Qualifications: Three years
providing mental health services as a clinical social worker,
mental health counselor or psychologist. Masters in Clinical Social
Work, mental health counseling Psychology or related field Current
Virginia state license as a mental health professional (LPC, LCSW,
LCP, or LMFT) Physical Requirements Able to lift & carry items up
to 15 lbs. 50% of the day. Able to use hands and fingers to operate
equipment such as a computer, tablet or smart phone. Able to stand,
