or Compact State License Position Summary: We are seeking a
Registered Nurse Case Manager to join the inpatient care
coordination team at Northwest Hospital, part of the LifeBridge
Health System. This role is responsible for comprehensive care
management, discharge planning, and coordination of services for a
diverse adult and geriatric inpatient population. Key
Responsibilities: - Provide holistic and individualized case
management and discharge planning for adult and geriatric
inpatients. - Collaborate with physicians, nursing staff, social
workers, and other healthcare professionals to develop and
implement patient-centered care plans. - Utilize evidence-based
practice to promote optimal patient outcomes and continuity of
care. - Evaluate patients for appropriate level of care, transition
planning, and resource utilization. - Educate patients and families
on post-discharge care and services, ensuring proper follow-up and
compliance. - Participate in interdisciplinary rounds, maintain
updated patient documentation, and ensure compliance with hospital
policies and regulations. Required Qualifications: - License:
Active RN license in Maryland or a Compact State. - Experience:
Minimum of 1 year of recent inpatient hospital case management
experience is required. - Solid clinical background in med-surg,
critical care, and/or emergency department settings. - Familiarity
with discharge planning, utilization review, and interdisciplinary
collaboration. - Certifications: BLS (Basic Life Support) required.
- Excellent communication, critical thinking, and organizational
skills. Preferred Qualifications: - Experience using Cerner and
Allscripts EMR systems. - Previous experience in a community
hospital or acute care setting with at least 40 daily census. -
Familiarity with common diagnoses such as diabetes, heart failure,
COPD, alcohol withdrawal, MI, respiratory failure, pneumonia,
COVID, etc. - Experience with post-operative surgical patients
(e.g., joint replacement, gallbladder procedures, wound care).
Schedule & Work Details: - Shift: Day shift (08:30 AM – 07:00 PM),
4 days/week. - Orientation: 72 hours; 16 hours are non-billable. -
Patient Ratios: 1 RN Case Manager to 20 patients (Days). -
Weekends: One weekend every fourth week. About HonorVet
